Authorization Form | Resting Waves

Please read the following carefully before signing.

I authorize Resting Waves LLC to receive, care for, transport, and scatter at sea the cremated remains of the person named above.

I confirm that I am authorized to make this decision for the decedent and, to the best of my knowledge, no one with the same or greater authority has objected.

I understand that once the cremated remains have been scattered, they cannot be recovered or returned.

I understand that the exact date, time, or location of the scattering may occasionally need to change because of weather, sea conditions, safety, or other circumstances. Resting Waves will always make every reasonable effort to complete the service as planned.

The cremated remains will be respectfully cared for and kept separately identified while in the possession of Resting Waves LLC.

Unless I request otherwise, I authorize Resting Waves LLC to respectfully dispose of the cremated remains container after the scattering has been completed.

I authorize Resting Waves LLC to photograph and document the scattering and record the date, time, and GPS location so that proof of service and a personalized Burial at Sea Certificate can be provided.

I authorize Resting Waves LLC to complete any required government notification or reporting related to the scattering.

If the cremated remains are mailed to Resting Waves LLC, they must be sent through the United States Postal Service (USPS). I will track the shipment and make every reasonable effort to make sure it is received safely. While the cremated remains are in transit, USPS is responsible for the shipment.
